If you're still registered at your old address:
Go to the Polling Place assigned to your old address.
If you are not sure where it is you can go to your county's Registrar of Voters website.
There you should find info on how to look up your polling place.
If the Poll Worker says they cannot find your name on the Registrar:
If you know you've voted in the last election, ask for a Provisional Ballot.
The poll worker cannot deny you a provisional ballot under any circumstances.
If you've voted by mail in the last election but have since moved:
Way to go idiot, you just gave whoever is living at your last address a EXTRA FREE VOTE.

Oh and on top of that, if the guy at your old address mailed your ballot in AND you cast a provisional ballot at a polling place as well, YOU are the person whose name is on both votes and now they have your new address to arrest you for voter fraud at. If you voted by mail last election and you were lazy since then, your punishment is NO VOTE FOR YOU. (or go and try to cast two, i dare you....)
